Clinical Track: LTCF Hospital Readmission Penalties & Strategies for Reducing Rehospitalizations

Beginning in 2019, LTFCs will be subject to rehospitalization penalties very similar to those affecting hospitals since 2013. This session will provide background information on this issue and outline the penalties for LTCFs. It will also include best practices for avoiding hospitalizations as well as strategies for treating in place in the LTCF.

In this interactive session the moderators will briefly share the current integration and evolution roadmap of RxPertise with the FrameworkLTC platform, and then open the floor for discussion. The goal is to understand what types of information participants would like to provide to customers to help validate the effectiveness and value of their clinical services. Participants will be invited to share their thoughts on the transition of care from the consultant pharmacist's perspective, the impact that short-term stays are having on pharmacy, and what tools would be helpful in managing those issues. The session will also seek feedback on the demands nursing homes are placing on pharmacies in order to help SoftWriters determine how to build automation around customers' current and future processes.
